Case Management Role for Youth Support

Join Phoenix Centre for Youth as a Case Manager and provide essential support to at-risk youth. Engage in advocacy, counseling, and resource facilitation every day.
In this position, you'll work with youth aged 16-24 to address their unique challenges related to housing insecurity. The ideal candidate will have a background in social work and a strong understanding of youth development. Your interventions will help create a supportive environment that prioritizes youth empowerment and well-being.
Key Responsibilities:
• Work closely with at-risk youth to create effective case plans
• Facilitate access to community resources and support services
• Maintain confidentiality while documenting assessments
• Contribute to program development and enhancement
• Foster a positive environment for youth engagement
