Position Summary
The Facilitator, IMPACT PLUS, utilizes in-depth clinical knowledge, educator skills, research, and clinical expertise to ensure that patients with complex co‑morbid conditions receive coordinated care in partnership with primary care practitioners at each designated Health Link.
