Your role will involve direct client engagement, health assessments, and coordination of care in partnership with multidisciplinary teams committed to integrated health services.
Key Responsibilities:

• Maintain detailed, accurate health records in EHR systems
Requirements:
• Graduate nursing degree with RN-EC certification
