At the heart of this transformation is our newly established Interface Care Division. This division is dedicated to driving bold innovation, strengthening integration between primary and secondary care, and placing patients at the centre of everything we do. Over the next three years, the division will lead ground‑breaking developments including the FNC+Plus care model, a large Virtual Hospital, Electronic Triage, and Rapid Assessment & Care (RAaC) units.
These roles are designed to transform care for heart‑failure patients by providing nurse‑led ambulatory care, virtual clinics and remote monitoring. Experience working with heart‑failure patients is essential. The team will work across all aspects of Cardiology and Heart Failure services, including ambulatory care, In‑Reach, Early Supported Discharge, Liaison and support at front doors and wards.
