Position Summary

This is a vacant position. The Nurse Practitioner (NP) is responsible for leading the provision of comprehensive geriatric services as part a collaborating member of an inter-professional Geriatric Assessment Intervention Network (GAIN) Community Team. The NP provides direct patient care focusing on health promotion, maximizing patient safety and function to support frail seniors living at home.
