Nurse Practitioner - Primary Care

Position Summary

The Primary Care Nurse Practitioner (NP) is a member of the Primary Care Team at ConnectWell Community Health, providing comprehensive primary health care services at the Lanark site and at least one day a week at the Carleton Place satellite location. The role emphasizes health promotion, client education, and self‑management, delivering services at the clinic and within the community.

Qualifications

• Completion of a recognized Primary Care Nurse Practitioner program
• Master's degree in Nursing or equivalent education and experience
• Current and unrestricted registration as an RN (EC) with the College of Nurses of Ontario
• Two years of experience in the nurse practitioner role in a primary care setting (community experience preferred)
