About the Role

• Provide high‑quality nursing care to patients in the community.
• Conduct comprehensive assessments and develop individualized care plans for patients with wound, ostomy, and continence concerns.
• Provide patient‑focused, safe, holistic nursing care and work collaboratively with team members.
• Ensure nursing care is based on current best practices to achieve successful outcomes.
• Continuously assess, anticipate, and respond to new and changing patient needs.
• Travel between patients’ homes within a specific geographical region.

Qualifications

• Minimum of two years of nursing experience (community nursing experience is ideal, but not required).
• Completed NSWOC or International Interprofessional Wound Care Course (IIWCC).
