Key Responsibilities
- Referral Management: Receive and process referrals from various sources, including healthcare providers, community organizations, and self‑referrals. Ensure timely follow‑up and management of incoming referrals.
- Appointment Coordination: Manage and schedule patient appointments with transportation, psychiatrists, counselors, therapists, and treatment care managers, ensuring optimal use of time and resources. Assist patients with transportation arrangements when necessary, ensuring they can attend their appointments.
- Coordination: Enter and maintain patient information in the system, ensuring accuracy and confidentiality at all times. Collaborate with therapists to ensure they are informed about patients needs and any schedule changes. <...
