• Schedule and assign caregivers for patients in homes or facilities based on patient needs and staff availability.
• Use platforms such as HHAexchange and/or Salesforce to confirm visits and maintain accurate scheduling records.
• Communicate clearly with caregivers, patients, and families regarding assignments, updates, and any scheduling changes.
• Track and document caregiver availability, eligibility, and assignment acceptance.
• Communicate effectively within our integral aide team and with diverse clientele in both English and Spanish.
• Maintain high standards of responsiveness and operational excellence.
