Position Summary: Provide transportation and peer support to treatment services for clients with opioid use and substance use disorder.
Essential Duties:
+ Collaborate with multi-disciplinary team to provide recovery support services.
+ Require driving to various community-based stakeholders across Long Island and other boroughs.
+ Field-based and on-site peer support to prospective clients.
+ Assist client with person-centered recovery goal planning.
+ Collaborate with client and care management team to identify the services clients should receive based on stage of change, immediate needs, and recommended treatment objectives.
