Description
Lead mental health initiatives as a Primary Care Network Clinician focused on Indigenous communities. Assess needs and implement culturally relevant treatment plans for clients.
This role in the Mental Health & Substance Use Services emphasizes the importance of connecting Indigenous clients with community resources. You will coordinate mental health assessments, provide immediate interventions, and support transitions to appropriate care within the PCN. Engage with both clients and their families to foster effective treatment strategies.
Key Responsibilities:
• Conduct diagnostic assessments and deliver short-term counseling
• Facilitate clients' access to rehabilitative services and programs
• Maintain detailed client records and documentation of services
• Advocate for Indigenous clients in securing housing and food support
• Educate staff and families about community mental health resources
Requirements:
• Master’s degree in Social Work or related field
