In this 14-month contract role, you will manage the execution of patient and caregiver support initiatives across Canada. With a focus on community engagement, you will identify opportunities for collaboration and work alongside healthcare partners to enrich the patient experience. This is a pivotal role helping to inform and enhance program delivery.
Key Responsibilities:
• Develop and maintain community partner relationships
• Enhance referral networks and program participation
• Monitor outreach effectiveness and access barriers
• Manage program delivery and documentation
• Prepare reports and insights for program planning
Requirements:
• 5+ years in community or outreach roles
