Social Work Professional Rehabilitation Focus Prince George

Transform lives as a Social Work Qualified with Northern Health in the Adult Rehabilitation & Recovery Services department. This position requires passion for client-centered psychosocial care and support. In this role, you will provide assessment, treatment planning, and advocacy for clients in need of specialized care.

You’ll work closely with a diverse team to enhance clients’ health outcomes through effective resources and strategies. This full time job follows a Monday to Friday schedule from 08:00 to 16:00.

Key Responsibilities

• Complete initial and ongoing client assessments
• Develop individualized care plans in collaboration with teams
• Deliver direct care and support to clients and families
• Educate and guide clients in self-management techniques
• Maintain administrative responsibilities and documentation

Requirements

• Bachelor’s Degree in Social Work from an accredited program
