Taykwa Tagamou Nation-Health Navigator/NHIB Service Worker

Taykwa Tagamou Nation is seeking a highly motivated, confident, organized professional individual to join the Health Department. Under the direction of the Director of Health, the Health Navigator/NHIB Service Worker will coordinate and deliver outreach strategies, identify community members, and work internally with fellow staff. They will also be responsible for ensuring assistance is provided in health navigation and support with the NIHB services, and will work to improve and enhance the quality of life for all ages in Taykwa Tagamou Nation.

Role & Responsibilities

• Navigate by assisting client with medical appointments: to improve client care outcomes, improve access, transitions, and guide with care planning, care pathways, health literacy support, digital access to care information, and assist clients in decision making to improve health outcomes.
