- American Heart Association certification is required for all direct patient care personnel
- Attach scanned copy of certification (with expiration date) to candidate's record in TrinityVMS
- Must include an image of the front and back of BLS card
- License/Certificate Verification must be dated within 30 days of start
- Iowa - 1860 mask only
- All engaged contractors must have page 27 signed form on file in Trinity VMS (Code of Conduct)
- Standard i9 completed by the agency
- Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement signed and on file
- Trinity Integrity and Audit Services Form signed within days prior to start
